Recycling totes must be recycled only

Terms: Net due upon receipt of the statement. Finance Charges, based on the following periodic rate of 1.5% per month (18% annually), will be applied to any past-due balance of 30 days or more. Prompt Payment of your account is Expected to continue services! All costs of collection including, but not limited to, a reasonable attorney's fee will be charged if this account is placed in collections.

Notice: If you do not have trash out at the time of service and you require us to return before your next scheduled pickup, a $5.00 charge will be applied to your account. A 96-gallon tote is provided for your use as a customer. Totes will be returned on termination of service. Otherwise, a charge of $75.00 will be added to your account. Anything over and beyond the tote will incur an extra charge, with prior permission from the office. Also, if your service is discontinued for non-payment on account, a $5.00 reinstatement fee will be applied to restart the service. The monthly charge is subject to change.

This is NOT a contract. Service is on a month-to-month basis. Once the form is completed, you will be contacted to provide payment info. and then placed on a route.
